The total bill came to 330 € for replacing those worn-out gears in the transfer case. I had nearly identical issues with my 1997 BMW 740d (32k km). Transfer case was definitely the culprit, the gear teeth were badly worn, causing that jerky motion and rough shifts you're describing.
