Thanks for getting back to me. I haven't checked the throttle position sensor specifically. Is that something that can be tested easily, or am I better off just replacing the accelerator pedal module outright?

Well, from my experience, similar symptoms ended up being a faulty accelerator pedal module. At that point I'd suggest taking it to a workshop.

Thanks for the insight! Turns out you were spot on. Took it to the shop, they confirmed the accelerator pedal module was defective, the repair cost me 205€. Runs like a dream now!

Hey, I've got an Avensis too, though I can't recall the exact year. Sounds similar to an issue I had a while back. Any chance you've checked the throttle position sensor?
