Berlingo Automatic Transmission Converter Fault

I've got a 2001 Citroen Berlingo diesel that's giving me fits. It's got about 220k on the clock, and recently the check engine light and transmission indicator light have both lit up. The gearbox slips sometimes, shifts feel unclean, and I'm hearing some nasty grinding noises when it does shift. Plugged it in and got an error code pointing to the automatic transmission converter. Anyone seen anything like this before? I am getting engine surge at highway speeds.

As for the slipping, it mainly seems to happen when I'm trying to accelerate onto the highway, especially around 50-60 mph. The grinding noise is more random, but often present when shifting from 2nd to 3rd gear. I can get the error code number to you later, once I get home.

Thanks a lot for the advice. I took it to a shop and you were spot on, converter fault confirmed. Ended up costing me 2120€ to get it sorted. Ouch! But at least it's running smoothly now.
