Fuel octane's a good shout. I've been using regular unleaded, but could bad fuel really cause all these symptoms? I mean, reduced power, jerking, smoke AND error codes? Is that even possible?

Yeah, it's possible, but unlikely to cause all those issues. Given the smoke and error codes, I'd strongly recommend a workshop. Could be something more serious than just fuel. Maybe cylinder heads or even ignition timing gone wild.

Turns out it was more than just bad fuel! The workshop found significant deposits in the combustion chamber. Cost me 425€ to get it sorted, including some engine tuning. Thanks for the advice!

Knocking in a Ford Explorer? Sounds familiar. I had something similar, though mine wasn't a hybrid. Have you checked your knock sensor or considered the fuel octane? Could be something simple... or not. What kind of fuel have you been using?
