Honestly, without the right equipment, interpreting that data can be tricky. Given your symptoms, and the risk of misdiagnosis, I'd recommend taking it to a reputable workshop. They can properly diagnose the issue, and replace the sensor if needed.

I also drive a GS 450h. I had a similar problem with the check engine light and the engine running rough. Did you check the fuel trims at all? What about the sensor readings? Might be worth a look before replacing parts.
