Lexus GX Mixture Preparation Error

I'm having trouble with my 2023 Lexus GX Hybrid at 186018 km. The engine shows signs of a mixture preparation error: reduced power, check engine light, rough running, jerking when accelerating, and stalls at idle. Spark plugs are fine, and I cleaned the throttle body, but no change. Could this be fuel injection or the intake manifold?

Sounds rough. I had similar symptoms on my old BMW 3 series: check engine light, power loss, the whole shebang. It turned out to be a defective injector.
