Happens mostly under load, climbing hills. Worse in higher gears. More detail needed?

Thank you for the additional information. The fact that the issue is more pronounced under load and in higher gears further supports the diagnosis of a defective torque converter lock-up clutch. Given the complexity of the automatic transmission system, I would strongly advise consulting with a qualified automotive technician for a comprehensive inspection and repair. Continuing to operate the vehicle in its current state may cause further damage.

I understand you are experiencing issues with your 2023 Lexus GX. The symptoms you describe, along with the stored error codes, do indeed suggest a potential problem with the automatic transmission's torque converter lock-up clutch. Could you provide any further details on the driving conditions when these symptoms occur? Does it happen under specific load conditions or during particular gear shifts?
