I've re-paired the phone multiple times. Could outdated software on the head unit be the problem, even with the phone OS current? Should I do a full system reset, or get diagnostic testing?

A full system reset could work, but with persistent pairing issues and a stored error code, it points to a deeper problem. Your RX probably requires professional attention. Are you prepared to start tearing your dash apart, or would you like a professional to have a look?

I brought my Lexus to the shop. The issue was an incorrectly paired device causing a software conflict. The repair involved recalibrating the Bluetooth module and clearing the error memory. Total was 45€ Thanks for the pointers!

Bluetooth gremlins, eh? Have you tried deleting the phone profile from the car and re-pairing? Is your phone's OS current? Sometimes those things cause headaches. Any other functionality affected?
