Vacuum leaks, okay. Where should I be looking specifically? I'm not super familiar with the engine layout.

Given all those symptoms, especially the smoke and hissing, a faulty EGR valve is the most probable cause. It's either stuck open or jammed. A replacement is likely needed. I'd get a professional diagnosis to be sure, but prepare for an automotive repair bill.

Just wanted to update. Took it to a workshop and they confirmed the EGR valve was toast. They replaced it, cleared the codes. Runs like new now. Thanks for the input!

Sounds like a classic EGR issue. Have you checked for any obvious vacuum leaks? Sometimes a loose hose can cause similar symptoms.
