Problems with acceleration and power loss in a Volvo C70 Diesel

Hello, I own a 2008 Volvo C70, diesel engine, with 81,000 km on the odometer. I've been experiencing problems with jerking during acceleration and loss of engine power. After the engine warms up, the accelerator pedal doesn't respond correctly and the car jerks. I have to accelerate very gently to get it to rev up again. At times, the pedal doesn't respond at all, and the car coasts, without any control. If I'm in 5th gear, I have to downshift to 4th to accelerate gently, but with jerks. The diagnostic scanner doesn't show any errors. I've been struggling with this problem for 4 months, I've replaced the high-pressure fuel pump and checked the injectors, but without any result. Does anyone have any idea what could be causing these problems?
