Thanks. Yes, it feels like it's struggling to breathe. The hissing makes me think vacuum leak, but could that cause all the other symptoms? It all started suddenly.

Yep, sounds like what I went through. I tried cleaning the EGR valve myself but it was a waste of time. Get it checked by a workshop, they'll know what to do. Save yourself the headache. Mine was a blocked EGR valve.

I had almost the exact same problems with my S40! Does it also feel like it's struggling to breathe, especially uphill? I'm no expert, but could you have a vacuum leak somewhere? That loud hissing sounds suspicious. I had rough idle too.
