I haven't pulled the fuel filter yet, but I did try a fuel system cleaner, thinking it might clear up any minor gunk. No luck. Is there anything else besides the fuel itself that could trigger that kind of sensor reading?

It's possible a defective cable or plug connection to the fuel composition sensor is causing a false reading. Given that the fuel cleaner didn't work, and you're still getting the code, I'd recommend getting it checked out by a workshop.

Took your advice and brought it to the workshop. Turns out it was indeed a faulty connection to the fuel composition sensor, just as you suspected. Repair cost me 95€. Thanks for pointing me in the right direction!

Incorrect fuel composition error on a newer hybrid Sequoia? That's odd. Have you checked for any obvious signs of fuel contamination, like water or debris in the fuel filter? Maybe a bad batch of fuel from a particular gas station?
