I had a similar issue with my Territory (though it's a slightly different year). Same engine. Reduced engine power and rough idle were the initial signs. Have you checked the fuel lines for any visible damage or blockages? A damaged fuel line could also cause similar symptoms and potentially trigger a fuel filter heater error due to pressure fluctuations, which would be detected by the engine control unit. Is the engine stalling?

Certainly. In my case, I initially focused on potential fuel line issues as well. When I mention electrical problems, it means that the fuel filter heater is a small electric heater warming up diesel to improve cold start performance. Considering the stored error code specifically mentions the heater circuit and you haven't found a mechanical issue, there is a strong chance your suspicion of a faulty connection is correct. Replacing it might solve it, but I'd suggest you consult a workshop for diagnosis and proper repair anyway. It might save time and further expenses.
