As for the slipping, it mainly seems to happen when I'm trying to accelerate onto the highway, especially around 50-60 mph. The grinding noise is more random, but often present when shifting from 2nd to 3rd gear. I can get the error code number to you later, once I get home.

Thanks a lot for the advice. I took it to a shop and you were spot on, converter fault confirmed. Ended up costing me 2120€ to get it sorted. Ouch! But at least it's running smoothly now.
