Cobalt check engine light and smoke

I'm having trouble with my 2012 Chevy Cobalt, gasoline engine. Lately, the check engine light has been on, and the car feels like it's lost a lot of power. When I really step on the gas, I'm seeing some smoke too. A friend suggested it might be something with the crankcase ventilation, maybe the PCV valve? Has anyone run into this before?

At that mileage, it's prime time for rubber components to degrade.
