Turned out to be a faulty transmission control module (TCM). Symptoms were almost identical to yours. Sadly, I couldn't fix it myself. My advice is to bite the bullet and get it diagnosed by a reputable workshop, and, if it is the TCM, get it replaced. There is not much to do yourself on this issue.

Thanks for the info! I was hoping it wouldn't come to this, but I suspected as much. I booked it into the local Lexus dealership. They confirmed it's a defective transmission control unit, and the estimated repair cost is 1000€. Ouch! At least I know what's going on now. Appreciate the help!

Sounds rough! I had a similar experience with my old Volvo V60. Paddle shifters went haywire, and the whole transmission felt 'off.' Is it safe to say that shifting feels like trying to stir concrete with a wet noodle?
