The car seems to be running mostly fine, maybe a slight decrease in fuel economy. What should I be looking for regarding the air-fuel ratio? Is there a way to test that without specialized equipment?

Slight decrease in fuel economy is a clue. Without proper tools, it's hard to nail down the air-fuel ratio. Given the error code, the exhaust leak, and the mileage (214426 km), it might be best to have a workshop take a closer look. It could be a simple fix with the cable or plug connection, and they can properly diagnose the sensor. You don't want to keep driving it with a faulty sensor; it could mess with your pollution control systems.

It sounds like a trip to the workshop is the best course of action. The cable/plug connection was defective. Repair cost was only 95€.

NOx sensor issues on a Lexus LS? Those sensors tell the car if it's polluting too much. An exhaust leak can mess with the sensor readings. Does the car seem to be running rough, or is your fuel economy tanking? If the air-fuel ratio is off, that could stress the sensor.
