Lexus LS uneven running issue

My 2007 Lexus LS hybrid is running unevenly at 241801 km. The check engine light is on with a stored fault, and I'm experiencing jerking during acceleration. Could a fuel pump or a dirty air filter cause this, or is it more likely an engine misfire or even the crankshaft sensor?

Thank you for the advice. It turned out to be a leak in the exhaust system, just as you suspected. Repair cost me 265€. Now it runs smoothly.
