Interesting. By vacuum system are you referring to the entire intake manifold and associated hoses? When you say exhaust system, do you refer to the bypass valve itself?

Affirmative to both. I had a cracked hose near the intake, causing a vacuum leak. My mechanic said that this caused the bypass valve to work overtime and trigger the error, maybe it's worth bringing into the workshop?

Understood. An inspection of the complete vacuum and exhaust system was performed. Turns out the error stored in error memory concerning the bypass valve was indeed a consequential error from another problem. Repair cost: 0€

I have a Lexus RC too. Although not hybrid, I had similar symptoms. Check your vacuum system and exhaust system for leaks. A small leak can trigger all sorts of weird errors. Think of it like a domino effect in your car's systems.
