Performance problems with my Ford Transit Diesel

Hello everyone, I own a 2013 Ford Transit with a diesel engine and 153,000 km mileage. I've noticed two problems: The engine runs rough and above 100 km/h the vehicle no longer accelerates as well. Has anyone had similar experiences or any tips on how I can fix these problems? Thanks in advance!
