Engine RPM seems normal, and I haven't checked the transmission fluid myself. Is a defective torque converter lock-up clutch a common issue on these?

While not exceptionally widespread, lock-up clutch failure within the torque converter is a known potential issue on Aisin-Warner transmissions utilized in several Volvo models, including yours. Given the symptoms and error codes, I strongly recommend a qualified workshop perform a thorough diagnostic inspection. Continuing to operate the vehicle in its current state may result in more substantial and costly damage.

Thanks for the insight. Took it to the workshop, and you were spot on. Faulty torque converter, repair cost 2120€. Replacing it fixed everything.

The symptoms you describe, combined with the error code, strongly suggest a fault within the automatic transmission's torque converter. Have you observed any changes in engine RPM during these slipping episodes? Also, has the transmission fluid been checked recently for discoloration or metallic particles?
